We have an exciting opportunity for a Nursing Associate to join our dynamic team. We are looking for someone who is enthusiastic about providing high quality care. The role will involve working with housebound patients, care home patients and contributing to PCN wide Population Health Management Projects. You will be based within Newark PCN which is made up of 7 GP Practices. Newark PCN has a patient population in excess of 79,000. You will be part of a wider team of Nurse Associates and be supported by the Practice Nurse teams, the PCN care home team and our PICS Lead Nurse for PCNs.
The role will involve delivering care to housebound patients in their own homes and care homes. The focus is on annual visits to patients to collect data for long term condition reviews but also an assessment of the impact of aging and illness on their life which includes assessing the risks associated with frailty such as falls, malnutrition and skin integrity. The postholder will undertake clinical tasks such as immunisation, ECGs and phlebotomy. The postholder will require a degree of flexibility to fit the needs of the service.

- GCSE Grade A-C in Maths & English or skills level 2 Maths & English or equivalent
