We are looking for a compassionate and experiencedAdvanced Nurse Practitioner to join our Home Visiting Service at Ashfield NorthPCN. This is a rewarding opportunity to make a real difference by providingtimely, high-quality care to patients who are unable to attend GP practicesettings across four practices in and around Sutton-in-Ashfield, serving apopulation of around 52,000.

In this role, you will assess, diagnose, and manage awide range of acute and chronic conditions through home visits and same-dayconsultations, delivering safe, effective, and person-centred care to those whoneed it most.

You will play an important part in supporting GPpractices by responding to triaged requests, helping to ease pressure onprimary care services and improving access for housebound and care home patients.The role also offers the chance to contribute to reducing avoidable hospitaladmissions and supporting proactive care for patients within frailty pathways.

As part of the Additional Roles Reimbursement Scheme(ARRS), you will be joining a wider multidisciplinary team across the PCN,working collaboratively to support integrated care and improve outcomes forpatients and communities.

You will work closely with GP practices, communityservices, and PCN colleagues, building strong working relationships whilemaintaining high standards of clinical practice, communication, and patientadvocacy in line with organisational and professional requirements.

Main duties of the job

  • Act as anautonomous advanced practitioner working independently and in conjunction withother health care professionals to assess, diagnose and treat the conditions ofpatients.
  • Avoidinginappropriate admissions and maintain patients in their home environments witha safe package of care and referral to other agencies as necessary, includingdirectly admitting patients to hospital when necessary.
  • Respondingto GP triage for home visiting both in the patients home and in care homes.
  • Provideevidence based professional advice to patients, carers and colleagues andensure the maintenance of clinical excellence and a proactive approach to care.
  • To developand lead new and innovative concepts, models, methods and practices to delivernew and improved primary care services to meet the needs of the practicepopulation.
