Primary Healthcare Darlington is seeking a committed GP to join the Enhanced Health in Care Homes (EHICH) team. You will deliver person-centred care to residents in elderly and learning disability care homes across Darlington PCN, leading MDTs and coordinating with pharmacists, care coordinators and ANPs.
You will champion evidence-based practice, contribute to clinical governance and quality improvement, and build strong relationships with care home teams and partner organisations.
