To work as part of the health and growth accelerator to help secure people support with employment. The role will support individuals to move from surviving to thriving by helping them to feel in control of their health and wellbeing by addressing mental health, general health, wellbeing, and inclusion needs using a behaviour change approach and active recovery methodology.

Primary Responsibilities

  • Provide individual support for people within primary care to explore ways of improving mental health and well-being.
  • Support people to utilise self‑help and to aid individual goal planning and measurement of recovery using standardised tools available on the clinical system as clinically relevant.
  • Support people to make informed choices about their health and wellbeing, which includes improving the levels of physical activity, promoting a healthier diet, and improving mental wellbeing.
  • Support access to wider community activities and provide support as part of an individual plan.
  • Effectively signpost and support access to appropriate services.
  • Support primary care and mental health colleagues to address the wider determinants of health and deliver an individual package of support and education.
  • Facilitate collective action, support and advice to people within local communities in order to reduce health inequalities while responding to identified health needs through one‑to‑one support.
  • Utilise social prescribing to develop individual recovery and support planning.
  • Participate in multidisciplinary working with other community health professionals.
  • Promote effective and efficient working practices across the team to achieve measurable outcomes.
  • Develop and maintain effective relationships with Talking Therapies leads, local community primary care, secondary mental health services, VCSE sector, and social prescribers.
  • Ensure performance information is collected accurately and timely to enable accurate and valid reports to be produced.
  • Actively support the team in fulfilling all contractual requirements of service delivery.
  • Safely and effectively manage own caseload under the supervision of a GP.
  • Monitor and evaluate: participate in formal evaluation of the role as applicable; collect relevant data on work activities and input it onto the patient-recorded database as required; be responsible and accountable to a supervisor from primary care.

Person Specification

Qualifications

  • Essential: Achieved NVQ Level 3 Health and Social Care or equivalent.
  • Educated to GCSE Level or Equivalent – minimum of 5 A‑E qualifications which must include Maths and English.
  • Evidence of a consistent pattern of learning from education, training and experience.

Experience

  • Essential: Significant experience of working within wellbeing, employment, or health services in a support worker or case manager role.
  • Knowledge and understanding of the factors that cause or impact on health problems and associated interventions.
  • Experience of working within a multidisciplinary team.
  • Demonstrated genuine interest in working with the service user group.
  • Desirable: Basic knowledge or understanding of the Mental Health Act and its use in practice.
  • Basic knowledge or understanding of the equivalent health or social care legislation appropriate to the service area.

  • Effective communication skills – written, verbal, non‑verbal.
  • Ability to work on own initiative without direct supervision, recognising where clinical input is needed.
  • Ability to work effectively without direct supervision.
