We are looking for an experienced and enthusiastic Advanced Clinical Practitioner (ACP) to join our friendly and supportive team at Lowther Medical Centre.
The successful candidate will provide high-quality, patient-centred clinical care, working autonomously to assess, diagnose, treat and manage patients with a wide range of acute and long-term conditions. You will work collaboratively with GPs and the wider multidisciplinary team, contributing to the delivery of safe, effective and accessible primary care.

NCPC (North Cumbria Primary Care) was established in August2019 in response to an increasingly fragile general practice provision withinNorth Cumbria. NCPCs vision is to bring practices together to develop asustainable, fully integrated primary care- built locally and deliveredtogether.
The Copeland NCPC area consists of 3 major practices,Fellview Health Care, Lowther Medical Centre and Seascale Health Centre as wellas a shared PCN (Primary Care Network) which delivers services to patientsacross all 3 practices.

Advanced Clinical Practitioner
Role Purpose
The Advanced Clinical Practitioners will work as part ofteam of diverse clinicians to provide access for patients and increase capacityand meet patient demand for appointments. The Advanced Clinical Practitionerwill see and treat acute patients and support the management of ongoing andchronic conditions.
As part of the wider Healthcare Team there is theopportunity for practitioners to bring or develop particular specialities whichsupport the health needs of our population. The post holder will work with theclinical and management team to ensure that staff and patients remain safe andservices are delivered in adherence to agreed standards, policies andprocedures.
Duties and Responsibilities
Clinical
Making professionally autonomous decisions

Provide comprehensive healthcare sensitive to patientsneeds, including assessment, diagnosing, prescribing, and referrals whereappropriate. Complementing the roles of GPs and the multidisciplinary team.
Conducting physical assessments to diagnose patientsconditions and provide appropriate self-care advice, prescribing treatment, orreferral using evidence based guidelines.
Direct and participate in provision of continuing care,health maintenance, screening and health promotion to patients as part ofpatient centred multidisciplinary team.
Managing and participating in Chronic disease management
Supporting and developing work towards agreed GMS targets.
Any other duties as may be required by the practice inproviding quality health care to patients.

Person Specification
Qualifications
- Masters degree in advanced clinical
- Practice Registered Nurse on the NMC register
- Non-medical prescriber
Other Experience
- Demonstrate high level of competence/advanced skills in clinical examination and diagnosis Interpret pathology/radiology results/reports and action where appropriate
- Excellent interpersonal and communication skills, providing enhanced discussions with patients and families regarding end of life wishes/planning
- Familiar with the complex geriatric assessment, 4AT, GPCOG Knowledge in EMIS clinical system Knowledge of the Accurx system
Experience
- Advanced communication skills
