An opportunity has arisen for an Adult or Paediatric Advanced Nurse Practitioner to join North Cumbria Primary Care (NCPC), with a position arising in Carlisle Locality. You will work independently, initiate effective history taking, clinical assessment and diagnosis within your capabilities. You will also implement a range of care interventions including prescribing for patients presenting with undiagnosed problems and/or support patients as part of a care plan.

Main duties of the job

  • The post holder is an experienced Advanced Nurse Practitioner who will work as a member of the Practice Clinical Team, assessing patients with undifferentiated and undiagnosed problems, initiate treatment, investigations, advice and management of problems using professional autonomous decision making processes, liaising when appropriate with other healthcare professionals and service providers to ensure safe and effective patient care.

Job responsibilities

  • Provide appropriate patient care in accordance with practice protocols and policies.
  • Actively engage patients and carers in management of their health, promoting health and wellbeing.
  • Work collaboratively with the multi-disciplinary general practice and primary care teams to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.
  • Be responsible for recognising, negotiating and implementing change processes to enhance the delivery of care, both within the Practice and when completing home visits.
  • Contribute to development and delivery of a high-quality innovative service to the patients of the practice within the community through the implementation of evidence-based clinical practice, supported by continual professional development, and provide support and mentorship to all healthcare professionals.

Person Specification

Experience

  • Experience in Primary / Secondary Care.
  • Experience in management of long term conditions e.g. asthma, COPD, diabetes, CHD.

Qualifications

  • MSc Advanced Clinical Practice.
