Health Care Co-ordinator in Appleby-in-Westmorland

We are looking for a compassionate, organised and proactive Health Care Co-ordinator to join our friendly general practice team.

Salaryup to £25,954 per annum, pro rata, dependent upon experience.

This is an important role supporting people living with frailty, helping them to receive coordinated, personalised care and remain independent in their own homes for as long as possible.

Working closely with GPs, practice nurses, community teams, social care and other members of the multidisciplinary team, you will help identify patients who may benefit from additional support and ensure that their care is well coordinated.

About the role

As Health Care Co-ordinator, you will:

• Working both remotely in the patients home and within the practice you will proactively support patients living with frailty and those at risk of deterioration.
• Work with patients and their families/carers to understand their individual needs, wishes and priorities.
• Contribute to the review of patients with complex needs and multiple long-term conditions, undertaking clinical observations and phlebotomy.
• Coordinate care between the practice, community services, social care, voluntary organisations and other relevant services.
• Help patients navigate health and social care services and access appropriate support.
• Support proactive management of patients with complex needs and multiple long-term conditions.
• Help identify patients at risk of hospital admission and support appropriate preventative interventions.
• Maintain accurate and up-to-date patient records and care plans.
• Follow up patients where appropriate and escalation concerns to the relevant clinician or service.
• Promote independence, wellbeing and shared decision-making.
• Support the practice in delivering high-quality, coordinated care to its frail and elderly population.

Job responsibilities

Carecoordinators play an important role within a PCN to proactively identify andwork with people, including the frail/elderly and those with long-termconditions, to provide coordination and navigation of care and support acrosshealth and care services.

Theywork closely with GPs and practice teams to manage a caseload of patients,acting as a central point of contact to ensure appropriate support is madeavailable to them and their carers; supporting them to understand and managetheir condition and ensuring their changing needs are addressed.

Thisis achieved by bringing together all the information about a personsidentified care and support needs and exploring options to meet these within asingle personalised care and support plan, based on what matters to the person.

Carecoordinators review patients needs and help them access the services andsupport they require to understand and manage their own health and wellbeing,referring to social prescribing link workers, health and wellbeing coaches, andother professionals where appropriate.

Person Specification

Experience

• Experience in working as a carer
• Experience of working in a health related sector
• Experience of multidisciplinary working
• Experience in working as a Health Care Assistant in general practice
• Local knowledge of VCSE and community services in the locality
• Knowledge of how the NHS works, including primary care

Qualifications

• Minimum GCSE (or equivalent) inc. Maths and English grade C;
• Driving license & use of a vehicle.
