An exciting opportunity has arisen to join our Primary Care Network (PCN) as a Project & Transformation Support Officer. This role provides high‑quality administrative and coordination support to the PCN Digital & Transformation Lead (DTL) and PCN Manager, helping ensure the smooth running of day‑to‑day operations, meetings, reporting, and communication across the network. The postholder will also support a newly funded £150,000 British Heart Foundation (BHF) 12‑month community cardiovascular prevention programme, coordinating across GP practices, community partners, and wider stakeholders.
Responsibilities
- Provide comprehensive administrative support to the PCN Manager and Digital & Transformation Lead.
- Coordinate meetings, prepare agendas, circulate papers, take minutes, and track actions.
- Support day‑to‑day coordination of requests and priorities.
- Maintain well‑organised digital filing systems and ensure documentation is up to date and accessible.
- Support the preparation of reports, presentations, and briefing documents for PCN leadership.
- Provide administrative and coordination support for the 12‑month BHF‑funded programme.
- Arrange stakeholder meetings with partners.
- Maintain programme documentation, trackers, and action logs.
- Support scheduling of engagement sessions and community activity.
- Assist in collation of basic activity data and reporting information.
