We are looking for an experienced dietitian to provide maternity cover within a Primary Care Network in Doncaster. This is an exciting opportunity to work at the forefront of primary care, delivering specialist dietetic services directly within GP practices.
As a PCN Dietitian, you will play a vital role in supporting patients with nutrition-related conditions, helping to reduce pressure on GPs while improving access to high-quality dietetic care in the community. This is a temporary contract.
Main duties of the job:
- You will manage your own clinical caseload, working autonomously within GP practices as part of a wider multidisciplinary team.
- Your focus will be supporting patients with conditions such as pre-diabetes, type 2 diabetes, weight management, and frailty - helping them make lasting changes through person-centred care and behaviour change approaches.
- You will also have opportunities to contribute to service improvement, clinical audit, and pathway development.
- Group education sessions may form part of the role depending on service needs.
