Primary Care Dietitians Ltd is seeking an autonomous PCN Dietitian to join a two-to-three GP practice network in Haverings, delivering specialist dietetic care within primary care. You will assess, diagnose and manage adults with nutrition-related conditions, including pre-diabetes, type 2 diabetes, weight management, frailty and malnutrition, supported by an MDT and GP colleagues.
