Wootton Vale Healthy Living Centre are looking for an enthusiastic and skilled practice nurse to join our friendly and forward-thinking primary care team. This position can be offered on a full or part-time basis with flexible options available. If you're passionate about providing high-quality patient care, enjoy working collaboratively, and have strong experience in long-term condition (LTC) management, we'd love to hear from you.
As a key member of our clinical team, you'll deliver a wide range of nursing services and support patients in managing chronic health conditions. We are particularly interested in someone who can take a proactive, holistic approach to patient care.
Main duties of the job:- Management and review of patients with long-term conditions (e.g., diabetes, asthma, COPD, hypertension)
- Supporting long-term condition clinics and QOF-related work
- Providing high-quality general practice nursing services such as immunisations, wound care, cervical screening, health checks and lifestyle advice
- Working autonomously while contributing to the wider multidisciplinary team
- Supporting practice initiatives to improve patient outcomes and access
About us: We are a friendly surgery with a list size of over 18,000 patients, over two sites, Wootton Vale, (MK43 9JJ), and branch site Shortstown Surgery, (MK42 0GS). We have good appointment access, which translates to happy patients, and a strong teamwork ethos, that includes an established clinical pharmacy team, GPs, nurses, physician associates, clinical liaison and admin teams. We are also a tier-2 sponsoring practice and would support any applicants requiring a tier 2 visa.

Person Specification:- Current NMC registration with an active PIN.
- Skilled in long term condition management with confidence running LTC clinics
- Knowledgeable in relevant clinical guidelines and best practice
- A great communicator with a compassionate, patient-centred approach
- Organised, proactive and able to work both independently and within a team
- Competence in managing and prescribing for LTCs
- Experience using GP clinical systems such as EMIS/SystmOne (if applicable)
