An opportunity has arisen for a clinical pharmacist to join South Uttlesford PCN, to provide high quality patient focussed pharmacy services to the patients of the PCN. This position is offered on a full-time basis and based in/out of a variety of South Uttlesford practices/locations according to need.

Main duties of the job:

  • Patient facing long-term condition clinics to review the ongoing need for each medication
  • Patient facing clinical medication reviews
  • Patient facing and telephone medicines support
  • Unplanned hospital admissions review of medications and reconciling of medications
  • Repeat prescribing
  • Identification of high risk patients

We formed our primary care network, to work together to enable us to operate more efficiently and ensure that a greater proportion of our resource is focused on clinical care and improved access. Our PCN covers 4 practices across West Essex, Peacock Surgery, Angel Lane Surgery, John Tasker House supporting 54,000 patients.

Person Specification:

Experience:

  • Strong experience within primary care
  • You will be required to lead by example, setting and maintaining high standards of professionalism within a team environment
  • You should be IT literate and confident in using practice systems

Qualifications:

  • As a minimum, you will hold a degree in Pharmacy (Masters or equivalent)
  • Be registered with the General Pharmaceutical Council and a member of the Royal Pharmaceutical Society
  • Ideally you will be a non-medical independent prescriber but the PCN will actively support you to achieve this, if not already an independent prescriber.
