Primary Care Careers is supporting Sudbury & West Suffolk Rural PCN with a part-time Social Prescriber role (16 hours per week) to link patients to community services and provide holistic support across Clare Guildhall and Siam Sudbury Surgeries.
The successful candidate will deliver one-to-one support, identify wellbeing needs, develop care plans and document outcomes, with NHS pension, pro-rata leave, induction, training and parking where available.
