The Colne Practice, located in Rickmansworth, are seeking an experienced practice nurse to join their busy practice, preferably with a respiratory interest (asthma, COPD, spirometry). This position is offered on a part-time basis, for 2 days per week, working a Thursday and a Friday. Working as a member of the multidisciplinary team, the successful individual will ensure nursing services are delivered effectively to the entitled patient population and implement processes for the management of patients with long-term conditions.
Main responsibilities will include:
- Asthma, COPD, spirometry
- Diagnose and manage acute and chronic conditions
- Provide wound care (ulcer/Doppler, etc.) to patients
- Develop, implement and embed an effective care management programme for the frail and housebound patients registered at the practice
- Assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice
- Develop, implement and embed health promotion and wellbeing programmes
- Implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing
- Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients’ health
About the Candidate
All applicants should have the following qualifications, registrations, skills and experience:
Essential:
- Registered Nurse (NMC)
- Experience of working within a primary care environment
- Experience of working autonomously
- Experience of infection control measures
- Clinical knowledge including; ECGs, venepuncture, wound care, women's health
Desirable:
- Mentor or teaching qualification
- Post graduate diploma or degree (CDM)
- Experience of working as a practice or community nurse
The Colne Practice is a friendly GP practice with a list size of approximately 10,500 patients. Our team comprises 7 GP partners, 2 practice nurses, 1 HCA, 2 clinical pharmacists and reception and admin teams. We have on site staff parking.
