West Norfolk Coastal PCN is recruiting a motivated Band 6 Tissue Viability Nurse (TVN) to support the delivery of a high-quality, evidence-based wound care service across all West Norfolk Primary Care Networks (West Norfolk Coastal PCN, Kings Lynn PCN, Fens & Brecks PCN, Swaffham & Downham PCN). This position is offered on a full-time basis, as a fixed term contract for 2 years initially.

Working under the clinical leadership of the Band 7 Tissue Viability Lead Nurse, you will play a key role in improving outcomes for patients with lower limb conditions and various complex wound aetiologies. Key responsibilities will include:

  • Provide specialist wound and lower limb assessments, including Doppler ABPI.
  • Manage leg ulcers, pressure ulcers and complex wounds using evidence-based care plans.
  • Initiate and monitor compression therapy (excluding bandaging) within defined protocols.
  • Support early identification and prevention of tissue breakdown.
  • Offer clinical advice to practice teams in line with ICB guidance.
  • Contribute to audits, data collection, and service improvement work.
  • Provide informal training and support to Practice Nurses, HCAs, patients and carers.
  • Maintain accurate clinical documentation and work collaboratively with multidisciplinary teams.

All applicants should have the following qualifications, registrations, skills and experience:

Essential:

  • First level registered nurse (adult)
  • Current NMC registration with an active PIN
  • Tissue Viability or Leg Ulcer Management Course
  • Degree level qualification or equivalent experience
  • Post-registration experience in wound care/tissue viability
  • Experience managing leg ulcers and compression therapy
  • Ability to travel across sites

Desirable:

  • Doppler ABPI competency (or willingness to complete)
  • Experience in Primary Care or Community setting

West Norfolk Coastal Primary Care Network (PCN) is a collection of independent GP Practices who have made a commitment to work collaboratively for the benefit of their patients. The PCN work on delivering high quality, innovative services through additional clinical and non-clinical roles across the coastal region of West Norfolk.
