SS9 South PCN has an opportunity for a care co-ordinator/healthcare assistant to join our team on a part-time basis of 30 hours a week. The role is based across Pall Mall and Highlands Surgery, Leigh on Sea. The post-holder will provide extra time, capacity, and expertise to our practice teams, helping us improve the care we provide to our patients.
Main duties of the job:
- Support SS9-South PCN in the delivery of high-quality primary health care projects and systems.
- Support the management teams and Clinical Director to ensure the smooth running of the PCN operations.
- Co‑ordinate attendance for the SS9-South PCN in meetings with external stakeholders, ensuring appropriate personnel attend in relation to PCN services.
- Co‑ordinate the delivery of seasonal Covid vaccination programmes including ordering, arranging mutual aid, and reporting to the ICB.
- Oversee the delivery of the Enhanced Access programme, to include setting rotas, sourcing and agreeing staff allocations.
- Overseeing the usage of appointments and undertaking any required reporting to the ICB.
- Support rota and allocations for the ECP teams and provide daily support for allocation of visits and route planning.
- Liaise with practices own administrators regarding PCN staff availability, ensuring PCN staff rotas are aligned and available within the practices.
- Support the SS9 South PCN and associated teams with delivery of services to housebound patients, which may include driving and chaperoning where appropriate.
- Provision of HCA support to housebound patients including phlebotomy, blood pressures, diabetic foot checks and other relevant services as required, within own range of competencies.

The practice shares a wealth of experience from GPs, physician associates, nurse practitioners, frailty team, ECPs, nurses, associate practitioners, HCAs and our phlebotomists. We also have a large, knowledgeable administration team, who work closely with our clinical team providing excellent patient care, along with a dedicated and supportive work environment. We are inclusive of ideas, which makes working here enjoyable and fulfilling. We operate a total triage system at the surgery, which enables us to make quick decisions about patient care and outcomes, ensuring the most appropriate appointment is given to each patient based on symptoms. We offer face‑to‑face (15‑minute) appointments and telephone consultations (10 minutes). We offer the NHS Pension Scheme and minimum 25 days FTE annual leave.
Qualifications:

Experience:
- Proven experience as a GP surgery or healthcare administrator.
- Excellent knowledge of SystmOne software.
- Strong analytical and reporting skills with meticulous attention to detail.
- Ability to effectively manage competing priorities and deadlines.
- Proficient in MS Office Suite (Word, Excel, Outlook, PowerPoint).
- Background in healthcare administration preferred.
- Excellent written and verbal communication abilities.
- Passionate about improving healthcare through technology.
- Ability to work as part of a team.
