Suffolk GP Federation have an exciting opportunity for a full-time advanced clinical practitioner (ACP) to join our well-established multidisciplinary team. You will work across all of our clinical sites in Haverhill, Long Melford, Lavenham and Glemsford, as well as our centralised Hub in Haverhill where clinical triage and remote consultations are undertaken.

Main duties of the job:- Deliver high-quality, patient-centred clinical care, including assessment, diagnosis, treatment and management of a wide range of acute and chronic conditions.
- Work autonomously within your scope of practice, including independent prescribing.
- Manage a mix of face-to-face, telephone and digital consultations, including triaged Hub-based work.
- Support the management of same-day demand and urgent care presentations.
- Undertake clinical triage using the eConsult system.
- Work collaboratively as part of a multidisciplinary team, including GPs, pharmacists, nurses, ECPs, PAs.
- Contribute to clinical governance, audit and service improvement activities.
- Support and mentor junior staff where appropriate, contributing to a positive learning environment.
You will be part of a highly supportive and collaborative multidisciplinary team delivering care to a patient population of over 28,000. Our team includes GPs, Clinical Pharmacists, Pharmacy Technicians, Nurses, Care Coordinators and other allied health professionals, all working together to provide high-quality, accessible care.
Person Specification:- Experience working in primary care or urgent care settings.
- Demonstrate strong clinical assessment and decision-making skills.
- Have excellent communication and interpersonal skills.
- Are adaptable and able to work across multiple sites and consultation types.
- Are motivated, proactive and committed to delivering high-quality patient care.
- Work well both independently and as part of a team.
- Are confident using, or willing to learn, new IT and digital consultation systems.
- Qualified Advanced Clinical Practitioner (ACP) with relevant MSc or equivalent experience.
- Registered with your relevant professional body (e.g. NMC/HCPC).
