Ludham and Stalham Green Surgeries have an excellent opportunity for a highly motivated, flexible and caring ANP or paramedic to join their friendly and supportive practice. This position is offered on a part-time basis, ideally for 3 or 4 days per week. We are flexible in terms of hours and this can be discussed at interview.
In this position, you will act as a first point of contact for patients requiring same-day, undifferentiated urgent care and will work closely with community nurses, GPs, and members of our expanding Integrated Care Teams.
Responsibilities- Diagnose and treat patients presenting with undifferentiated primary care demand.
- Triage and treat patients wishing to see a health care professional, making any necessary referrals to other members of the primary health care team.
- Make professionally autonomous decisions for which you are accountable.
- Provide first point of contact within the service for patients presenting with undiagnosed problems, using skills in history taking, physical examination, problem-solving and clinical decision-making to establish a diagnosis and management plan.
- Provide health education/health promotion to patients, relatives and carers.
- Refer patients to other members of the primary and secondary health care team as necessary.
- ECG (Lead II interpretation).
- Minor injuries including wound care.
- Admission avoidance work.
- Pathological specimens and investigatory procedures.
- Undertake the collection of pathological specimens.
- Perform investigatory procedures either clinically indicated or requested by other members of the primary health care team.
- Participate in the education and training of students of all disciplines and the introduction of all members of staff where appropriate.
- Maintain continuing education by attendance at courses and study days as deemed useful or necessary.
- Work with the wider multi-disciplinary community integrated teams.
- Establish and maintain effective communication pathways with colleagues, patients and their carers.
- Excellent PN, HCA, Social Prescriber, first-contact Physio, Clinical Pharmacist (as well as other ARRS roles)
- Large and supportive management, clinical and administrative team

- Collaborative working with other local practices within a very pro-active PCN
