Buckhurst Hill, Loughton and Chigwell PCN has an opportunity for a Care Coordinator to join our team on a full-time basis, to support our GP practices and to assist in re-shaping primary care across our communities. The role involves working very closely with the practices and the multidisciplinary team (MDT) within the PCN, and is pivotal in ensuring that all patients receive the best possible care and service.

The main duties of the role include:

  • Working with GPs and other professionals within the practice to identify and manage a caseload of patients, particularly working with vulnerable patient groups with long-term conditions that may require this extra support.
  • Using in-depth knowledge of locally available support and services to develop care plans with patients, focusing on prevention.
  • Supporting the coordination of all key activity, including access to services, advice and information, as well as ensuring that health and care planning is timely, efficient, and patient-centred.
  • Helping patients prepare for, or follow-up after, clinical conversations.
  • Supporting digital initiatives.
  • Using clinical systems on a daily basis, with confidence to run searches and keep care records up-to-date.

If you are a skilled Care Coordinator, with excellent person-centred planning experience and strong local knowledge for accessing key support, then we would love to hear from you. The ideal applicant will have the following skills, experience and qualifications:

  • Experience of working within healthcare, the voluntary or community sector, to support vulnerable groups.
  • Diploma Level 2 in Health and Social Care, or an equivalent qualification (desirable not essential).
  • Familiar with local resources and services, including how to access them.
  • Motivated by helping people, with care and empathy.
  • Able to deliver person-centred support in a non-judgmental way.
  • Experience of completing holistic person-centred care planning assessments.
  • Strong listening and communication skills.

We are a training practice, serving a patient population of 12,000. We operate a SystmOne clinical system with a history of high QOF achievement and a CQC rating of good across all domains.
