Walton Medical Centre are looking to welcome an experienced advanced clinical practitioner/advanced nurse practitioner to their team on a full-time basis. The successful applicants will be able to demonstrate safe, competent clinical decision-making and deliver expert care, including assessment, diagnostic skills, treatment and evaluation of patients who wish to be seen by a healthcare professional, presenting with minor illness and/or their continuing medical and nursing needs.
Main duties of the job:
- Acting as an expert practitioner, demonstrating advanced clinical competence and a knowledge base beyond those associated with conventional nursing roles.
- Working as an autonomous clinical practitioner/nurse, providing high quality and timely patient centred care.
- Independently prescribe, working within the current guidelines, reviewing, and monitoring patient medications in accordance with the formulary.
- Delivering face to face/telephone/e-consult/video consultations as well as home visits.
- Directing care appropriately and managing complete episodes of care independently.
- Undertaking consultations with patients of all ages including children as an autonomous practitioner and using own clinical judgement, diagnose, treat, refer and/or discharge patients.
- Working within and across the MDT teams and signpost/draw upon expertise of all members to support and meet the patients' needs and best interests.
- Recording clear and contemporaneous consultation notes to agreed standards, ensuring accurate read coding.
- Participating in, and contribute to, all clinical targets (QOF, QIS, ES etc).
- Recommending, instigating, and explaining appropriate invasive and non-invasive diagnostic tests, investigations and interpret findings and reports.
About us:
Walton Medical is based in the seaside town of Walton on the Naz in north-east Essex. There is a wide socio-economic and demographic served within our area which will provide an interesting and varied case mix. You will be joining an enthusiastic team of clinicians and administrators. The clinical team consists of 2 Partners, 2 salaried GPs, ANPs, Practice Nurses and GPAs. You will be well supported with on-going professional development (training support, CPD and peer support). The role will contribute to the improving quality of care of our patients of the practice.
Person Specification:
Experience:
- Post-registration experience at a senior level (Band 7 or above) in specialty area.
- Evidence of on-going CPD.
- Experienced, caring nurse who can demonstrate highly developed clinical skills and is able to demonstrate critical thinking, whilst working within their professional boundaries.
- Act as an autonomous advanced practitioner able to work independently as well as in conjunction with other members of the clinical team.
- Advanced clinical assessment skills.
- Ability to perform in stressful and complex situations.
- Sound clinical reasoning & decision making.
- Flexibility.
- Ability to inspire and lead a team.
Qualifications:
- Professional registration with relevant professional body: NMC, HCPC or GPhC.
- Post-graduate qualification in specific speciality, or equivalent experience.
