Overview
Are you a clinician with telephony triage experience? Primary Care 24 (PC24) is excited to share the position of Bank Urgent Care Practitioner (UCP).
Salary: £24.50-£31.85 per hour
Location: Wavertree Technology Park, Wavertree, Liverpool
Shifts: 08:00-13:00, 13:00-18:00, 18:00-23:00, 23:00-08:00 (Friday evening to Monday morning)
Responsibilities
- Liaise with nominated medical / nursing leadership to provide oversight and coordination of patients awaiting contact.
- Manage clinical demands in collaboration with the Clinical Coordinator / Shift Manager to ensure delivery of safe care within agreed waiting times.
- Escalate concerns / priority patients to the Clinical Coordinator / On-call teams.
- Review patients awaiting contact to provide oversight and minimise clinical risk by actively filtering for potential signposting to more appropriate services.
- Operate around a Red Flag approach of triage, prioritising patients in line with service escalation plans including the elderly, palliative patients and those at risk of deterioration.
- Apply clinical skills and reasoning to ensure patients are transferred safely between different clinical dispositions and signposted when required for safe clinically appropriate disposition.
- Provide skilled and effective assessment of patients presenting clinical need through telephone consultation, using professional clinical judgment with the support of clinically based algorithms.
Qualifications
- Extensive experience in emergency health care settings.
- Significant expertise in clinical triage.
- Degree in Clinical Practice.
- Minimum 3 years post-qualification experience.
- Clinical Examination and Diagnostics Level 6 / 7 or relevant telephony triage experience.
- Experience of working in urgent care / primary care or similar setting.
