Do you have customer service and administration experience? We are looking for a Care Navigator/Medical Receptionist, which is a dual role in our Primary Care Hub and GP practices. You will provide excellent customer service to patients, carers, healthcare professionals and other services when making calls on behalf of PC24 GP practices and do so in a caring and compassionate manner.

Salary: £18,067.55 per annum

Location: Garston Family Health Centre

Contract: Permanent

Shifts: Various shifts Monday to Friday between 27.5 hours per week

Main duties of the job:

  • Provide care navigation call handling duties on behalf of the PC24 practices, in line with PC24 processes and procedures.
  • Open & close premises, complete security checks and ensure all clinical rooms are prepared.
  • Ensure through effective communication that all calls are directed to the most appropriate clinician and/or service, identify and escalate any emergency callers to appropriate ambulance services.
  • Ensure that all patients and visitors are greeted professionally & politely and attendance is logged through the system.
  • Deal with any questions and queries that arise.
  • Provide administration support to the wider primary care team and action and follow up calls to other services as required.
  • Provide operational support to the onsite practice team and clinical team.
  • Record all essential information in a concise and accurate manner using EMIS Web system processes.
  • Liaise with other services to request any instructions given by the clinical team, ensure all information logged.
  • Understand local referral arrangements/pathways for holistic support from a wide range of sectors.
  • Ensure patient confidentiality at all times.
  • Be familiar with and adhere to PC24 Standard Operating Procedures.
  • Be patient focused, ability to show patience, empathy and compassion via the telephone and face to face.

What do I need:

  • Excellent verbal and written communication skills.
  • Flexibility to work as a team and independently.
  • An understanding of the importance of recording accurate data in a confidential manner.
  • Great IT skills including all Microsoft package experience; experience of EMIS is also desirable.
  • Previous experience in call handling and reception positions.
