Do you have customer service and administration experience? We are looking for a Care Navigator who will provide excellent customer service to patients, carers, healthcare professionals and other services when making calls on behalf of PC24 GP practices and do so in a caring and compassionate manner.
Salary: £16,425.05 per annum
Location: Primary Care Hub, Litherland
Contract: Permanent
Shifts: Monday – Friday 08:00–13:00 (25 hours per week)
Responsibilities:
- Provide care navigation call handling duties on behalf of the PC24 practices, in line with PC24 processes and procedures.
- Ensure through effective communication that all calls are directed to the most appropriate clinician and/or service, identify and elevate any emergency callers to appropriate ambulance services.
- Provide support to the wider primary Care team and action and follow up calls to other services as required.
- Provide operational support to the onsite practice team and clinical team.
- Record all essential information in a concise and accurate manner using EMIS Web system processes.
- Liaise with other services to request any instructions given by the clinical team, ensure all information logged.
- Respond to circumstances with a positive and flexible approach.
- Understand local referral arrangements/pathways for holistic support from a wide range of sectors.
- Ensure patient confidentiality at all times.
- Be familiar with and adhere to PC24 Standard Operating Procedures.
- Be patient focused, show patience, empathy and compassion via the telephone.
Qualifications:
- Excellent verbal and written communication skills.
- Flexibility to work as a team and independently.
- Great IT skills including all Microsoft package experience; EMIS is also desirable.
- Previous experience in a call handling position.
