An exciting opportunity has arisen for an enthusiastic and motivated Practice Nurse to join our vibrant, forward-thinking practice. The successful candidate will be an integral part of the nursing team and will be responsible for participating in the delivery of high-quality patient care to our registered practice population. Working alongside our Nurse Manager, three Practice Nurses, and two GPAs; we are ideally looking for chronic disease experience in Diabetes Management. This post is a permanent position at the Practice, 20-24 hours per week.

Main duties of the job

The focus of the role is both the delivery of evidence-based practice for patients presenting with a long term condition, and the provision of preventative health care to the practice population. You will deliver care, focusing on supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities. They will work collaboratively within the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.

Job responsibilities

  • To be responsible for diagnostic procedures for example, venepuncture, blood pressure, height/weight, ECG and any specimens.
  • Work with other health care professionals to diagnose, monitor, manage and treat long-term conditions, including non-drug-based treatment methods using a management plan, and in line with national and local policies and practice needs.
  • To advise patients on general health care and promotion of healthy lifestyles.
  • To be responsible for chronic wound care as well as post-surgical wound care.
  • To administer injections under an individualised prescription or PGD within training limits.
  • To maintain the cold chain of vaccines, including safe storage, rotation and disposal of vaccines and drugs.
  • To administer immunisations for example, influenza, pneumococcal, tetanus and childhood immunisations.
  • Support and manage health needs of women presenting for family planning and cervical cytology consultations.
  • Advise, support and administer vaccinations where appropriate for patients travelling abroad.
  • Assist in providing minor-surgery sessions.
  • Apply infection-control measures within the practice according to local and national guidelines.

Core Duties

  • To work independently and as part of a busy Nursing Department, to meet the needs of patients and service delivery.
  • To be an active and integral member of the Nursing Team, attending team meetings and training sessions, supporting colleagues and working flexibly to cover annual leave or sickness.
  • To act as chaperone if required/requested by patients and colleagues.
  • Support the development of others in order to maximise potential.
  • Actively promote the workplace as a learning environment, encouraging everyone to learn from each other and from external good practice.
  • To assist in maintaining a safe environment for the clinical team, patients and co-workers, ensuring that all statutory and local regulations (e.g. Health & Safety, COSHH, clinical waste management, infection control) are implemented and followed.
  • To ensure data is recorded accurately and timely onto patient records, with due regard for record management and patient confidentiality.
  • To ensure data is accurately recorded and coded onto relevant templates and documentation associated with patient health care.
  • To maintain and stock examination rooms and treatment rooms with agreed consumables and clinical equipment/instruments.
  • To be aware of quality standards and targets as set out in NICE guidelines, NHS contracts, Quality and Outcomes Framework, etc., working to achieve these standards at all times.
  • To promote a professional approach at all times by attire and attitude to patients and staff both in and out of the Practice.
  • To maintain annual registration with the NMC and other professional bodies (as required). To adhere to the Code of Professional Conduct.

Person Specification

  • Minimum of 1 years recent experience working in primary, community or hospital nursing experience, as part of multi-disciplinary team.
  • Experience in managing long-term conditions (e.g. diabetes, asthma, COPD, hypertension).
  • Experience in childhood and adult immunisations.
  • Wound care and dressings.
  • Knowledge of infection prevention and control procedures.
  • Understanding of safeguarding children and vulnerable adults.
  • Experience of working autonomously within professional scope of practice.
  • Competence in cervical cytology.
  • Experience with chronic disease review templates and QOF.
  • Experience using primary care clinical systems (emis).

Qualifications

  • Registered Nurse with NMC Registration.
  • Evidence of continued professional development.
  • Minimum of 1 years recent experience working in primary, community or hospital nursing experience.
  • Post-registration qualification in Primary Care Nursing.
  • Care of the Adult with Diabetes qualification.

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.

£25,391 to £27,531 a year dependant on experience, pro rata.
