Your Role at Prestige Nursing & Care
We are looking for an experienced Care Planner with solid planning and rostering experience, matched with a flexible, enthusiastic and can-do attitude, to join our Dartford team.
Previous experience as a Care Planner / Care Coordinator within a domiciliary/home care setting is essential.
You will assist and support the Branch Manager in ensuring key business and compliance targets are achieved, while also supporting recruitment to aid branch growth.

The Care Planner is responsible for promoting Prestige Nursing & Care while ensuring the effective utilisation of nursing and care resources, aligning staff availability with client requirements while maintaining the highest standards of quality, compliance and safety.
Responsibilities
- Ensuring all client visits are covered on a daily basis.
- Organising Quality Visits for the Field Care Supervisors and Field Care Managers.
- Liaising with Branch Nurses to support assessments, training and competencies for complex care packages.
- Providing excellent customer service to both internal and external stakeholders.
- Liaising confidently with healthcare professionals, social workers and case managers.
- Ensuring compliance with all office systems, policies and procedures.
- Taking ownership of your allocated area and working collaboratively with other Care Planners to ensure care is fully covered.
- Communicating with frontline colleagues regarding changes to clients’ care needs.
- Maintaining excellent relationships with existing clients, carers and colleagues.
- Identifying new business opportunities alongside the Branch Manager to support branch growth.
- Working with multidisciplinary teams to maximise opportunities to meet clients’ care needs.
- Ensuring the effective placement of frontline colleagues to meet client requirements.
- Preparing ad hoc reports as required.
- General Responsibilities
- Supporting the wider team with day-to-day responsibilities when required.
- Working confidently using your own initiative when lone working.
- Championing excellent internal customer service.
- Undertaking any other reasonable duties requested by your manager.
Essential Experience (Must Have)
Previous experience working as a Care Planner / Care Coordinator within a domiciliary/home care setting is essential.
- Experience using electronic care planning and rostering systems.
- Experience managing complex care rotas and responding effectively to last-minute changes.
- Knowledge of working within a CQC-regulated care environment.
- Experience liaising with carers, clients, families, local authorities and healthcare professionals.
- Strong understanding of compliance, safer recruitment and care sector regulations.
- A full UK driving licence and access to a vehicle is desirable but not essential.
