Key Responsibilities:

  • Lead and oversee clinical components of complex care delivery
  • Support assessments, mobilisation and implementation of care packages
  • Conduct risk assessments and develop person-centred care plans
  • Deliver clinical training, competency assessments and supervision to carers
  • Liaise with MDTs, commissioners, case managers and families
  • Promote best practice, safeguarding and clinical governance
  • Maintain high standards of care quality and compliance
  • Support branch growth and development of clinical services
