The Role
As a Community Link Worker you'll be at the heart of helping people access the support they need. Often the first point of contact for individuals and partners, you'll build trusted relationships and provide personalised support. More than signposting, you'll work alongside people to understand their goals, develop tailored support plans, and guide them through every stage of their journey.
Key Responsibilities
- Carrying out person-centred, holistic conversations by phone or in person
- Developing outcomes-focused support plans and providing practical support
- Working with statutory and voluntary sector partners to coordinate support
- Updating, maintaining and monitoring records
- Identifying advocacy needs and safeguarding concerns, taking appropriate action
- Processing referrals and answering enquiries
- Building positive relationships and trust with service users and partners
About You
- 1β2 years' experience in social care, health, community services or VCSE
- Experience supporting people with complex or multiple needs
- Understanding of the barriers faced by adults at risk in North Northants
- Strong relationship-building skills across sectors
- Committed to EDI and trauma-informed practice
- Organised, digitally confident, and able to manage a caseload
- Full driving licence and access to a vehicle (essential)
