The Team Leader is an integral part of the support team, responsible for meeting the practical, physical, and emotional needs of the individuals we support within the service. Through a key worker system, they will design individual support programmes and coordinate the delivery of these packages by a group of Support Workers and themselves.

About The Role

  • Plan, coordinate, and implement the safe transition of new individuals into the service and community where necessary.
  • Liaise with the families of the people we support and relevant members of multi-disciplinary teams, community groups, and voluntary bodies to promote their needs.
  • Plan, develop, and implement individual and group programmes of support to enhance and maximize the capabilities and independence of the people we support in the service.
  • Coordinate ongoing assessment and review regularly through the completion of suitable assessment models.
  • Support the physical, emotional, and personal needs of the people we support, ensuring that work is individual-focused and upholds the principles of respect, privacy, dignity, fulfilment, independence, and choice.
  • Ensure that medication is held, stored, and administered according to Praxis Care's Medication Policy and current statutory and legislative procedures.
  • Provide effective leadership, supervision, and management to Support Workers, including monitoring time and attendance and participating in any capability, disciplinary, and grievance procedures.
  • Participate in learning and development as necessary within the organization while identifying and addressing personal development opportunities for themselves and Support Workers.
  • Ensure that adequate records are created and maintained, and necessary reports are produced in line with organizational policy and statutory requirements.
  • Support the manager with regulatory compliance concerning all regulating bodies.

Requirements of the Role

  • Participate in the company appraisal and staff review process.
  • Adhere to and model the company values, behaviours, and competencies at all times.
  • Engage in formal and informal supervision as required.
  • Carry out all duties in accordance with Praxis expectations, regulations, and policies.
  • Take on additional duties of the role and other required duties as necessary.
  • Work the necessary hours to fit the needs of the service.
  • Where the role requires sleep-ins, a minimum additional payment of £45.00 per night applies.

Essential and Desirable Criteria

  • Education, Experience & Training Qualifications Essential: 2 years experience in a health and social care setting and a commitment to undertake a professional qualification, OR Qualification in: Social Work, First Level Nursing, Allied Health Profession, Level 5 Diploma in Leadership and Management for Health and Social Care, OR 1 year paid experience in HSC setting with a commitment to undertake a professional qualification and a Psychology Degree.
  • Desirable: Experience in mental health or learning disability services.
