The Team Leader is an integral part of the support team, responsible for meeting the practical, physical, and emotional needs of the individuals we support within the service. Through a key worker system, they will design individual support programmes and coordinate the delivery of these packages by a group of Support Workers and themselves.
About The Role
- Plan, coordinate, and implement the safe transition of new individuals into the service and community where necessary.
- Liaise with the families of the people we support and relevant members of multi-disciplinary teams, community groups, and voluntary bodies to promote their needs.
- Plan, develop, and implement individual and group programmes of support to enhance and maximize the capabilities and independence of the people we support in the service.
- Coordinate ongoing assessment and review regularly through the completion of suitable assessment models.
- Support the physical, emotional, and personal needs of the people we support, ensuring that work is individual-focused and upholds the principles of respect, privacy, dignity, fulfilment, independence, and choice.
- Ensure that medication is held, stored, and administered according to Praxis Cares Medication Policy and current statutory and legislative procedures.
- Provide effective leadership, supervision, and management to Support Workers, including monitoring time and attendance and participating in any capability, disciplinary, and grievance procedures.
- Participate in learning and development as necessary within the organization while identifying and addressing personal development opportunities for themselves and Support Workers.
- Ensure that adequate records are created and maintained, and necessary reports are produced in line with organizational policy and statutory requirements.
- Support the manager with regulatory compliance concerning all regulating bodies.
Requirements of the Role
- Participate in the company appraisal and staff review process.

- Engage in formal and informal supervision as required.
- Carry out all duties in accordance with Praxis expectations, regulations, and policies.
- Take on additional duties of the role and other required duties as necessary.
- Work the necessary hours to fit the needs of the service.
- Where the role requires sleep-ins, a minimum additional payment of £45.00 per night applies.

Essential and Desirable Criteria:
Education, Experience & Training Qualifications
- Essential: 2 years experience in a health and social care setting and a commitment to undertake a professional qualification, OR Qualification in: Social Work, First Level Nursing, Allied Health Profession Level 5 Diploma in Leadership and Management for Health and Social Care, OR 1 years paid experience in HSC setting with a commitment to undertake a professional qualification and a Psychology Degree.
- Candidates must be prepared to complete Level 2 Safe and Effective Care Practice Diploma or already hold registration with NISCC/NMC or an Allied Health Professional.
- Desirable: Experience in mental health or learning disability services.
