As a Support Worker, you will help people live happy, independent lives. This may include:
- Supporting people with daily tasks, accompanying them to appointments, encouraging social activities, helping people develop new skills, and supporting them to be active members of their community.
Key Accountabilities:
- Supporting People in Their Daily Lives: Help individuals with everyday tasks such as preparing meals, shopping, tidying their home, and managing household routines. Encourage people to do as much as possible for themselves, promoting confidence and independence.
- Encouraging Social Activities and Community Involvement: Support individuals to take part in hobbies, interests, clubs, and social activities they enjoy. Help reduce social isolation by supporting participation in outings, events, and community activities.
- Building Positive Relationships: Develop supportive, professional relationships based on trust, respect, and kindness. Listen to individuals and support them to express their views, wishes, and goals.
- Keeping People Safe: Be aware of potential risks and concerns and report these appropriately. Follow company policies and procedures to help ensure a safe environment for everyone.
- Recording and Sharing Information: Keep accurate and clear notes about support provided. Communicate effectively with colleagues and managers to ensure consistent support.
- Working as Part of a Team: Work positively with colleagues, families, healthcare professionals, and other partners involved in a person's support.
Requirements of the Role:
- Participate in the company appraisal and staff review process.
- Adhere to and model the company values, behaviours, and competencies at all times.
- Carry out all duties of the post in accordance with Praxis expectations, regulations, and policies.
- Be prepared to take on additional duties and responsibilities as required.
- Work the necessary hours to meet the needs of the service.

Essential Criteria: Numeracy and literacy skills, effective communication skills.
