Band 8a | Inside IR35 | Hybrid | London – 2 days on site / 3 days Remote

Start: July 2025 | Duration: Until End of September 2025

Are you a dynamic project manager with a passion for integrated care? An exciting short-term opportunity has opened to support the continued development of five well-established neighbourhood teams in one of the Outer London Boroughs.

You will be working closely with local health, council and community partners, you’ll drive forward collaboration, coordination and delivery – ensuring all teams continue their journey toward maturity and shared outcomes for the local population.

Key aspects of the role:

  • Provide programme coordination and leadership across all Integrated Neighbourhood teams, supporting progress and accountability
  • Facilitate multi-agency meetings and co-ordinate agreed actions
  • Strengthen working relationships across their partners -primary care, the council, community, mental health and acute services
  • Develop or refine frameworks or a programme plan to track team development and effectiveness

What we’re looking for:

  • Strong project management background delivering NHS transformation in multi-agency settings
  • Deep understanding of system wide working – across health, local government and voluntary sectors
  • A track record of influencing and engaging stakeholders at all levels
  • Experience or knowledge in integrated or neighbourhood care models

Nice to have:

  • Background in primary care transformation
  • Knowledge of integrated approaches to children’s services
