We have an exciting opportunity for a Release and Transfer Nurse Coordinator to join our Primary Care team at HMP Wormwood Scrubs , a Category B prison located in London.
In this role you will provide the highest standards of healthcare to our patients, without compromising on efficiency or quality.
HoursMonday to Friday: 07:30 – 15:30
SalaryThis full‐time position offers an annual salary up to £55,086, depending on experience.

Main duties of the job
Follow and implement the Release and Transfer Pathway
Oversee the management of assessments of health needs, planning, delivery and evaluation of care, including clinical risk assessments
Manage face‐to‐face clinics with patients and act as the experienced clinician for patients who may need additional support for their release/transfer
Contribute to MDT discussions and work collaboratively with other healthcare professionals and prison colleagues for safer discharge planning
Line manage other members of the Transfer & Release team and collaborate with release teams in the prison and outside organisations

Requirements
NMC Registered Adult Nurse
Evidence of implementing local and national policies (e.g. NICE guidance, developing protocols/care pathways and monitoring effectiveness)
Experience demonstrating high levels of clinical expertise
Line management experience
Experience in a hospital, community or prison environment (not essential but ideal)
Legal and security checksApplicants must have the legal right to work in the UK and will be subject to additional vetting and security checks, including a Disclosure and Barring Service check under the Rehabilitation of Offenders Act (Exceptions Order) 1975.
UK registrationApplicants must hold current UK professional registration.
