We have an exciting opportunity for a Patient Pathway Coordinator to join our friendly team at Practice Plus Surgical Centre, Gillingham. As the first point of contact in our patient pathway, you’ll play a vital role in ensuring a smooth, efficient and supportive experience for every patient — from referral through to treatment. This is a role where your attention to detail and organisational skills will truly make a difference to patient care and outcomes.

What you\’ll be doing

As a key member of our administration team, you will:

  • Manage patient referrals, pre-assessment and clinic bookings, ensuring all information is accurate and complete.
  • Liaise with GPs, consultants, and patients to coordinate appointments and arrange transport or translation services where required.
  • Provide a professional, compassionate, and courteous service to patients, whether over the phone or face-to-face.
  • Handle patient and clinical queries efficiently, ensuring confidentiality and sensitivity at all times.
  • Update and maintain patient records on our systems, ensuring accuracy and compliance with data protection.
  • Support clinical teams by processing clinic outcomes, booking follow-up appointments, and ensuring smooth scheduling of surgical procedures.
  • Play a proactive role in helping the team meet targets and maintain excellent service delivery standards.

What we’ll look for in you

As an experienced administrator with strong IT skills (Word, Excel, and data entry), you will also be:

  • A confident communicator with excellent interpersonal skills and a natural ability to put patients at ease.
  • Highly organised, with the ability to manage competing priorities and work effectively under pressure.
  • A true team player who can also work independently with minimal supervision.
  • Flexible, positive and committed to delivering high-quality patient care.
  • Previous experience in a healthcare setting or an understanding of medical administration would be an advantage, but not essential.
