Join Practice Plus IUC as an Advanced Nurse Practitioner (ANP) or Advanced Clinical Practitioner (ACP) within the Suffolk and North East Essex Unscheduled Care Coordination Hub (UCCH), an innovative multidisciplinary service focused on improving patient outcomes, avoiding unnecessary hospital admissions and keeping care closer to home. Based in Ipswich, the hub brings together clinicians, paramedics and community partners to support patients with urgent care needs.
Working alongside colleagues from urgent care, community, mental health and wider healthcare services, you will use your clinical expertise to assess patients, coordinate appropriate care and help ensure patients receive the right support in the most suitable setting. This role is particularly suited to ANPs and ACPs with experience in primary care and independent prescribing.
A key part of the role involves completing remote consultations, reviewing urgent care cases and supporting timely clinical decision-making. You'll receive training to develop your telephone consultation skills and contribute to the Suffolk and North East Essex Clinical Assessment Service when required.
As a valued member of a collaborative multidisciplinary team, you'll play an important role in service development, quality improvement and patient safety while representing Practice Plus IUC with professionalism and compassion.
