Practice Plus Group is seeking an experienced Prescribing Pharmacist to join our Devon Integrated Urgent Care (IUC) and Clinical Assessment Service. In this role, you will provide pharmacy leadership and support within the Clinical Hub, working as part of a dynamic multi-disciplinary team to deliver high-quality, evidence-based care that meets the needs of patients across Devon. You will play an active role within a cross-disciplinary, multi-skilled team, helping to ensure service quality aligns with corporate goals and strategy. You will also contribute to the development of innovative models of care that positively impact the wider health economy. Working closely with the Regional Manager, Pharmacy Lead, Medical Lead, and the wider clinical team, you will help ensure the service consistently meets its contractual and clinical obligations.
Responsibilities
- Providing safe and effective telephone advice on medicines use and pharmaceutical care to clinicians, patients, and carers
- Prescribing appropriately, including repeat medications, in line with agreed service parameters
- Offering clear self-care and medicines management advice
- Managing and implementing medication changes following hospital discharge or outpatient appointments
- Assessing and managing requests relating to:
- Repeat prescriptions
- Medication queries
- Health information
- Minor ailments and onward referral
What we’ll look for in you
You will be a GPhC-registered Prescribing Pharmacist with strong clinical judgement and experience in medicines management, including repeat prescribing and post-discharge medication changes. You’ll be confident providing telephone-based pharmaceutical advice and prescribing in line with Practice Plus Group policies and local and national guidelines. You’ll take a patient-centred approach, offering clear self-care advice and knowing when to signpost patients appropriately. Strong communication skills and the ability to work both autonomously and collaboratively within a multidisciplinary team are essential, along with a commitment to quality, safety, and governance.
