Our Health in Justice service line is now expanding into non-Custodial healthcare, a person-centred approach delivered via a solid and consolidated infrastructure aimed to assess individuals’ mental health and neurodiversity requirements, to redirect them to the most appropriate channel. We have an exciting opportunity for a Clinical Reconnect Practitioner to join our team in West Yorkshire.
Hours – Monday to Friday; 8am – 6pm
Some weekend working may be required
Salary – This position is full time and you will receive a salary of £43,609 to £49,175 depending on experience
Location – West Yorkshire Prison Sites – HMP Leeds, HMP New Hall & HMP Wealstun & Community

Candidates are required to have a Legal right to work in the UK due to the nature of the role, offers of employment will be subject to additional vetting and security checks.
What you\\\’ll be doing
As a Clinical Reconnect Practitioner your responsibilities will vary. You will
- you will work autonomously in prison and community, to identify and respond to requests of screening and assessment of people with identified health needs leaving the prison establishment within a 12-week period or less.
- The aim of the service is to reduce reoffending by addressing the health and social needs of the service user and ensuring onward referrals are made to ensure continuity of care provision. This role will work closely with probation and other key stakeholders to ensure the needs of the service user are effectively assessed and addressed, avoiding duplication.
- Working autonomously, you will be required to ensure the needs of the service user are met and communicated to relevant parties to ensure continuity of care, onward support and effective risk management.

Qualifications And Skills Required
- Qualified Registered Mental health Nurse or Social Worker with current registration with the relevant professional body.
- Experience of working with patients, face to face, who have complex mental health conditions
- Awareness of substance misuse services and support available
- Sound clinical knowledge and assessment skills including risk assessment
- Understanding of the complexities of multi-disciplinary and multi-agency working
- A non-judgmental and compassionate approach
