About The Role
Unlock your best work life and make a difference to every life. We have an exciting opportunity for a Release and Transfer Clinical Lead to join our Primary Care team at HMP Brixton, a category C Prison located in London.
Hours: 37.5 hours per week, Monday to Friday, 08:00-16:00 or 09:00-17:00.
Salary: This position is full-time; you’ll receive an annual salary up to £63,119 per annum, depending on experience.

Candidates are required to have a legal right to work in the UK on a permanent basis and, due to the nature of the role, offers of employment will be subject to additional vetting and security checks.
What you’ll be doing
Your responsibilities will vary, but you will:
- Hold complex case release meetings and attend prison release meetings to ensure that an adequate handover has been given
- Work alongside the Mental Health Clinical Lead to deliver a seamless service for our patients
- Implement the pathway that creates a smooth pathway for our patients back into the community or transferred to another establishment
- Oversee the management of assessments of health needs, planning, delivery and evaluation of care for the prison population, including clinical risk assessment

Qualifications and experience required
- Qualified Adult Nurse with NMC PIN
- Evidence of implementation of local and national policies e.g. NICE guidance, developing protocols/care pathways and monitoring effectiveness
- Experience demonstrating high levels of clinical expertise in decision making and judgement
- Knowledge of clinical audit and its relationship to research
- Line management experience
