We have an exciting opportunity for a Bank Long Term Conditions Nurse to join our Primary Care at HMP Brixton, a category C Prison located in London Borough of Lambeth, in inner-South London. With numerous training and development pathways, free parking on site and easy links to central London, HMP Brixton offers a great and rewarding working environment!

This is a bank role working ad hoc shifts.
Salary
This position is bank; you’ll receive an hourly rate of £25.14.

Candidates are required to have a legal right to work in the UK on a permanent basis and due to the nature of the role, offers of employment will be subject to additional vetting and security checks.
Main duties of the job
As a Long Term Conditions Nurse, your responsibilities will vary. You will:
- Be responsible for ensuring a comprehensive handover of services when patients are transferring across wings or custodial care settings.
- Identify immediate health needs including any external services or prescribing requirements that prisoners may have received e.g. prior to remand or repatriation onto a wing.
- Facilitate arrangements between agencies involved in delivering a complex care package to ensure effective collaboration and stability for the patient in delivery of that care.
- Provide proactive intervention to urgently address improvement recommendations from CQC and HMPPS inspections.
- Deliver continuity of care across health and social care agencies from courts, to custody and community whilst improving the communication between the above.
- Inspire the continuous improvement of MDT working across HMP and healthcare teams.

Qualifications
Registered Nurse- General with relevant NMC Registration
Advanced Nurse Prescriber/ Practitioner
Educated to degree level
Knowledge of health and social care models across the criminal justice system and of national and local initiatives relating to physical health
Knowledge of Evidence Based Practice and Clinical Governance
Experience of assessing the needs of and working with individuals with complex health and social care needs.
