We're looking for experienced Qualified Social Workers who are committed to putting people at the heart of everything they do.The RoleAs a Hospital Social Worker, you’ll work at the frontline of care planning and discharge, making a real difference in the lives of vulnerable adults. With your skills and expertise, you'll support timely and safe discharges, helping individuals regain independence and reduce their risk of harm.You’ll:
  • Carry out person-centred assessments in a hospital setting.
  • Use your professional judgment to create effective support and care plans.
  • Work collaboratively with NHS colleagues, families, and other professionals.
