Pertemps Social Care are currently recruiting for an experienced Adult Social Worker to join a hospital-based team in London on a full-time basis. We are looking for an experienced social worker who can operate at Senior Practitioner level and has strong previous experience working within a hospital discharge setting. You will work closely with health professionals, service users, families and other agencies to support safe, timely and appropriate hospital discharges.
Key Responsibilities
- Undertake complex Care Act assessments, reviews and support planning.
- Manage hospital discharge cases, ensuring individuals are discharged safely and appropriately.
- Work collaboratively with NHS colleagues, OTs, health professionals, care providers and families.
- Assess care and support needs and identify appropriate packages of care, reablement and community support.
- Manage safeguarding concerns and complex cases where required.
- Apply professional judgement when managing risk and making decisions around discharge.
- Maintain accurate and timely case records in line with statutory requirements.
