We are currently recruiting for an experienced Preparing for Adulthood Case Officer (14–25) to join Newham's SEND service. This is an excellent opportunity to support young people aged 14–25 as they prepare for adulthood, helping them achieve positive outcomes through effective transition planning. You will manage a caseload of Education, Health and Care Plans (EHCPs), leading annual reviews and coordinating person-centred planning that promotes independence, further education, employment, training, and community inclusion.
Working closely with young people, families, education providers, adult services, and partner agencies, you will ensure that EHCPs remain outcome-focused and support successful transitions into adult life.
Key Responsibilities- Manage a caseload of young people aged 14–25 with EHCPs.
- Lead annual reviews and Preparing for Adulthood planning.
- Develop outcome-focused, person-centred EHCPs.
- Work collaboratively with families, schools, colleges, adult social care, health professionals, and external agencies.
- Support young people to access education, employment, training, independent living, and community opportunities.
