Contract role, Community Wellbeing Teams

Pertemps are recruiting for experienced Adult’s Social Workers in the East Riding of Yorkshire. We are looking to appoint an agency Social Worker to support the Community Wellbeing Teams across East Riding of Yorkshire Council, initially supporting the Wolds Community Wellbeing Team and Holderness Community Wellbeing Team. There is an expectation of flexibility to travel across the area.

The role will involve undertaking:

  • Care and Support Assessments
  • Care and Support Planning Reviews
  • Mental Capacity Assessments and Best Interest decisions
  • Safeguarding enquiries (Section 42)
  • Supporting hospital discharges where appropriate

Visits will need to be completed face to face, with the option of hybrid working. Please get in touch for more information.
