Job Description

Adult’s Social Worker, Single Point of Access Team, North Lincolnshire Pay rate – to £35 per hour Contract role, Hospital Discharge Team Pertemps are recruiting for experienced Adult’s Social Worker in the North Lincolnshire area. This is working as part of the Single Point of Access Team which will include  home visits and completing Care Act Assessments, working within the hospital integrated discharge team based at Scunthorpe General Hospital.   Completion of MCA/BI as necessary . Please get in touch for more information. This is an agency post and Pertemps can offer you:-
