Pertemps are recruiting for an experienced Adult’s Social Worker in West London. An exciting opportunity has arisen to join Specialist Services in the London Borough of Hammersmith Fulham. We are looking to recruit three Specialist Services Social Workers with a range of experience in adult social care, especially with delivering purposeful reviews in line with project work requirements and Care Act 2014.
We are transforming the way we deliver our services within the borough. This is an opportunity to bring your ideas as well as your experience to a dynamic social work team. We are looking for enthusiastic social workers who can assess the social care needs of service users and their carers under the Care Act, to provide direct social work intervention with service users within the borough.
Our vision within integrated health and social care is focussed on implementing new ways of working in personalisation and a strength-based approach to enable individuals to live as independently as possible to make informed choices and control over their lives and the care that they receive. This will require applicants to be able to confidently assess and implement the management of risk in partnership with other agencies. Working knowledge of MOSAIC is essential.
