Adult Social Worker – Hospital Team

We are currently recruiting for an experienced Adult Social Worker to join a Hospital Team in Merton.
The role will involve working with adults in a hospital setting, supporting assessment, discharge planning and positive outcomes for individuals returning to the community or moving into appropriate care.
Key responsibilities:
- Undertake Care Act assessments and reviews
- Support hospital discharge planning
- Assess care and support needs and develop person-centred plans
- Work collaboratively with health and social care professionals
- Safeguard adults and respond to identified risks
- Liaise with families, carers and partner agencies
- Maintain accurate records and work within statutory timescales
Requirements:
- Social Work England registration
- Qualified and experienced Adult Social Worker
- Experience within hospital or discharge services is desirable
- Strong knowledge of the Care Act and adult safeguarding
- Ability to manage a busy caseload effectively
