Prevention of admission services in Hertfordshire provide acute and community-based care to help patients avoid unnecessary hospital stays.
Key services include:
- Hospital at Home / Virtual Wards
- Urgent Community Response / Rapid Response
- Hertfordshire Admission Avoidance
The team covers South and West Hertfordshire, including the localities of St Albans, Dacorum, Watford, Hertsmere, and Three Rivers.
