This is an ideal opportunity for a Registered Nurse who is seeking to broaden and utilise their range of skills and knowledge to nurse people in their own environment as part of a multi-disciplinary team. The Community Staff Nurse is responsible for the delivery of prescribed nursing care to patients on a day-to-day basis. This would include the holistic assessment of patient care needs and the development, implementation and evaluation of evidence-based care. Duties include:
- Palliative care support and assessment
- Care at the end of life
- Supporting and assessing individuals and carers' needs with long-term health conditions
- Promoting self-management skills
- Wound care
The post holder will be autonomous in their clinical practice while acknowledging their limitations and actively seeking advice as necessary. The Llanfair Welshpool District Nursing Team are an integrated part of the virtual ward that aims to prevent unnecessary admissions to hospital and support early discharge from hospital. There are various hours available up to 30 hours due to realignment of the service.
Main duties of the job:
- Responsible for providing support to the District Nursing Team in the day-to-day management of multidisciplinary/multi-agency packages of scheduled and unscheduled care for patients with multiple, complex and palliative care needs.
- Responsible for prioritising their own delegated home visits according to the patients' clinical need and geographical location.

Person Specification:
- Other: Ability to travel between sites in a timely manner, excellent organisational skills, ability to deal with complex/stressful situations, local knowledge of statutory and voluntary services.
- Aptitude and Abilities: A high level of interpersonal and communication skills, ability to engage with members of the public, ability to work independently and as part of a team - to include the wider MDT, ability to speak Welsh.
- Experience: Evidence of a wide range of clinical skills, competence and confidence in a wide range of clinical skills including: palliative care, chronic disease management, wound management.
- Qualifications and knowledge: NMC Registered Nurse, evidence of continuing professional development, working towards diploma/degree/masters qualification in CHS (District Nursing) Specialist Practitioner DN, working towards supplementary Nurse Prescriber qualification.
