Key Information
- Permanent, 15 hours per week
- North Powys ward‑based team
- Work environments include community hospitals, virtual wards and patients’ homes
Responsibilities
- Deliver near‑patient medication services on the ward, including stock management and patient medication assessment
- Electronic ordering of medicines
- Confirm medication histories and reconcile medicines with patients
- Counsel patients about new medicines
- Collaborate with clinicians, district nurses, mental health team, therapists, pharmacists and specialist nurses
- Develop and implement SOPs and safe medicine systems
- Facilitate effective patient discharge and provide post‑discharge support
- Work with clinical pharmacists, multidisciplinary team members and primary care colleagues
Qualifications and Experience
- Registered pharmacy technician, preferably holding ACPT qualification and Medicines Management accreditation
- Pre‑registration technicians due to register in the next few months are welcome to apply
- Experience in a community hospital or ward‑based setting is desirable
Additional Requirements
- Ability to speak Welsh is desirable; English or bilingual applicants are welcome.
