The predicted salary is between 30000 - 42000 £ per year.

The role is central to the co‑ordination of both complex and non‑complex discharges. The post will be based within the Patient Flow Hub, and has responsibility for overseeing non‑complex and complex patient discharges within PTHB Community Hospitals.

In the course of their duties the candidate will be responsible for:

  • Facilitating and/or assisting with effective, efficient and timely discharge of patients from hospital, with particular emphasis on quick turnaround of patients.
  • Providing complex multi‑disciplinary assessment signposting, liaising directly with patients and their families, Care Transfer Coordinators and MDTs, and assisting in case conferences for patients with complex health and social care, palliative care or rehabilitation or housing needs.
  • Giving families support and information.
  • Supporting the patient flow hub and working closely with a range of professionals and external agencies within the hospital and in community services.
  • Working independently, following procedures and treatment plans, but under the direction of the patient flow hub, and within agreed boundaries.
  • Providing ongoing/complex planning regarding process mapping of patient journeys for modelling purposes.
  • Working effectively with the wider multi‑disciplinary team and third‑sector groups to facilitate patient discharges in a timely manner across Powys, supporting ward staff and multi-disciplinary teams in facilitating effective, safe, and timely patient discharge.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
