About the role

Post is a 23 hour per week Support Worker. Working to a six‑week rolling rota with the required working hour of 5 weeks at 24 hours and 1 week at 18 hours. The rota is subject to change to meet the needs of the service. There is a requirement to work shifts day and evenings as well as weekends and Bank Holidays. You will be supporting adults who have a Learning Disability to manage their own home as per the principal responsibilities detailed in the Job description.

What you will do

  • Help people with learning disabilities to develop their skills and independence by demonstrating and modelling activities.
  • Work alongside a dedicated staff team and other professionals in the service users’ own home and other required venues.
  • Undertake practical tasks and support people with their personal care, domestic activities and social activities in the community.
  • Support service users to build and maintain family and friendship links.
  • Support service users to reach goals and achieve personal outcomes.
  • Participate constructively in regular staff supervision and appraisal sessions.
  • Contribute to written assessments and Care Plans, attend Care Planning meetings, compile and keep detailed records and reports.
  • Participate/attend required training to develop, grow and gain skills to enhance the role.

This position has a requirement for an Enhanced DBS Check.
